Consultation & Referral Information
Referring a client and patient
We want to make the referral process as easy and stress-free as possible for you and your client. It is best to have your client call to make their appointment so that it best fits their schedule. Feel free to call to discuss the patient with us ahead of time, especially if you have specific concerns that you would like addressed.
In order that we might serve your client best, it is important that we have as much information as possible at the time of the patient's appointment (preferably 24 hours prior). A referral form can be downloaded from this website. Please print and fill out as completely as possible and send with the client or fax to us. Alternatively, a written summary of the patient's history is acceptable if you prefer. In addition to recent written records and copies of pertinent laboratory test results, any images (radiographs, ultrasound images, CT/MRI images) should come with the patient. It is also possible to e-mail images to us at peakvetspecialists@q.com.
Phone Consultations
Our specialists are available for phone consultations with referring veterinarians as time permits. Please have all pertinent information on hand when you call, including patient signalment, history, and diagnostic findings. For involved cases, it may be most efficient to fax pertinent records prior to the consultation call. There is no charge for phone consultations for referring veterinarians.
Dr. Lowry and Dr. Van Camp are available for radiographic reviews and radiology rounds. For written or verbal reviews you may drop off radiographs at their office at 820 Gilgalad Way in Fort Collins. Radiographs may also be dropped off at our clinic in Windsor for review during Dr. Lowry and Dr. Van Camp's regular office hours on Monday, Wednesday, Thursday and Friday. There is a fee associated with verbal or written reviews.
